We are excited to celebrate your baptism with you. Before completing this form please read the baptism booklet that can be found here or at the Info Center. Then complete this form to confirm your desire to be baptized on Friday, April 12, 2019. Please submit this form no later than Sunday, March 10, 2019.

Recording your Story

We would like to capture your story on video prior to the baptism service (see questions below). When you are baptized we will play the video of you telling your story. Please select a time and date that you would be available to record your story at Blackhawk Church Brader Way (the recording will take about 15 mintues). You will be contacted with a finalized time by March 12. If you have any questions, please contact Ashley Moore, amoore@blackhawkchurch.org. 
Everyone who is baptized will receive a DVD of the baptism service.:

Profession of Faith

I have come to a place in my life where I fully commit to trust and follow Jesus Christ as my Lord and Savior now and forever.

Your Story

The purpose of this section is to help you articulate your journey of faith. Portions of your story will be captured on video being told by you, prior to the baptism service as noted above.
*Question 1: Do you believe you will have eternal life in God's presence? Why or why not?
*Question 2: Please state briefly how and when you accepted Christ as your Savior. The answer to this question will be the focus of your recorded video. Here are some questions that will help you formulate your story.
1. What was your life like before you accepted Christ? Where did you find your security? How did those things let you down?
2. How did you come to Christ? Why did you decide to accept Christ?
3. How is your life different now?

Tips:
Pray before you write out your story. 
Write the way you speak.
Be Honest.
Practice your story out loud several times until you feel comfortable with it.

Video Release

I grant permission to Blackhawk Church to use my photo or video in print, future Sunday messages, on the website, or on social media. All baptisms will be videotaped.
